I'll leave the pictures visible if they're smallish (<640x480px, <100kb) and cut anything larger. (Except when I forget, oops.) I also don't hot-link as most sites don't appreciate (and many even block) that; instead, I rehost at a free site (photobucket in my case) and hyperlink the image to the originating page.

I try to remember to use smallish versions, and cut if there are lots (not a hard-and-fast thing, I know). These are my own photos, at Flickr, which makes it easy to do the small-version-of-larger-picture thing.
I think I hotlinked the last time I used a photo not my own, but that was at least a year ago.
If there's to be more then one shot I usually put one pic out of the cut as a teaser since I'm looking to entice people to look beyond the cut.
I use a fairly narrow view on my LJ so typically if it doesn't break my LJ its good on most other people's.
If the shot is one I took I host it on flickr, if its something else I host it in my LJ gallery. I try not to hotlink (especially outside of a cut) but if the pic is on a photo sharing site (flickr, photobucket etc) obviously hosting is what they are for. I will occasionally hotlink to a site that's selling something but always with a link to the sale site; if I'm advertising for you you can bloody well host the pic of your product for me.

Depends on size and number -- may cut, or more often use thumbnails or medium-sized versions that link to larger ones, but for a single medium-sized image I may just leave it directly visible.
I don't use pictures on my LJ at all. On my costume blog, I show pictures I have taken (they live in my Picasa account), and sometimes hotlink elsewhere
